PRIMARY FUNCTION/PURPOSE

The Psychiatric Nurse Practitioner provides medical and psychiatric care of the clients in the transitional Housing Program(s).

PRIMARY RESPONSIBILITIES

  • Provide triage and initial health and mental health assessment(s) of clients presenting with medical and psychiatric problems.
  • Provide psychiatric care as needed, including psychopharmacology and psychotherapy.
  • Provide crisis intervention and assessment as needed.
  • Prescribe medications, reviews with clients’ side effects and monitors client’s response to prescribed medications.
  • Consult psychiatrist in determining treatment plans as needed.
  • Respond to emergencies and ensures follow up with emergency services and follow-up care.
  • Advocate and liaise with medical providers in obtaining medical and psychiatric care in the community.
  • Provide clinical supervision to the Licensed Practical Nurse (LPN).
  • Use motivational interviewing, recovery and trauma-informed approach when delivering services to clients.
  • Collaborate with the social services team in assessing clients’ needs, developing a service plan and coordinating care for the client(s).
  • Participate in team meetings, case conferences, staff meetings, etc.
  • Organize workshops for clients and staff on health topics as needed.
  • Support staff in providing trauma informed services.
  • Document delivery of services, clients progress, etc. in electronic case record in accordance with agency policies and procedures.
  • Perform other duties as assigned.

MINIMUM QUALIFICATIONS

  • Bachelor’s degree in nursing required with at least one (1) year of experience in a related field and a minimum of two years supervisory experience preferred.
  • NYS Registered Nurse.
  • Experience with the mentally ill, homeless or related field desirable.
  • Excellent analytical, evaluative, and human service management skills.
  • Excellent written and oral communication, and organizational skills.
  • Must have incredible initiative, the ability to work well under pressure, and the ability to work both independently and as part of a team to complete complex projects in a fast-paced environment.
  • Must be adept and comfortable with crisis management.
  • Advanced level of computer proficiency with proficiency in Word, Excel, and the ability to learn new technologies and databases.

OTHER REQUIREMENTS (including Physical Demands)

  • Travel required.
  • May be required to work long hours and a varied schedule to meet job demands and deadlines.
