As a PMHNP with MindCare Solutions, you will collaborate with interdisciplinary teams to deliver high-quality, value-based psychiatric care across a variety of clinical settings. • Conduct Comprehensive Psychiatric Evaluations: Perform thorough psychiatric assessments to establish a baseline, manage psychotropic medications, and deliver personalized, need-based care.
• Ongoing Medication Management: Develop and adjust treatment plans, including medication management, based on patient progress and input from the interdisciplinary team.
• Collaborative Care Planning: Partner with case managers and interdisciplinary teams to create and refine care plans, ensuring a seamless, integrated approach to behavioral health management.
