Psychiatric Nurse Practitioner

Job Summary

Strive Health is seeking a full-time Nurse Practitioner to provide psychiatric evaluations, medication management, and follow-up care for residents in nursing homes and assisted living settings. Responsibilities

• Conduct initial psychiatric assessments and routine follow-up visits
• Prescribe and manage psychotropic medications
• Collaborate with facility staff, medical directors, and care teams
• Document in EMR in a timely and accurate manner

Qualifications

• Must have a current and active NP license and DEA # in the State of Wisconsin
• Must have at least one year experience working as a Nurse Practitioner
• Willing and able to commute to long term care facilities
• Experience with geriatric populations or interest in working with older adults
• Caregiving, patient-centered care philosophy
• Strong communication and collaboration skills

Pay

Competitive base + productivity bonus (commensurate with experience)

Schedule

Monday to Friday (flexible hours)
