About the role

The Psychiatric Nurse Practitioner will provide direct psychiatric services for homeless individuals at three separate, general population treatment programs who are not connected to care.

• Document medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, and outcomes.
• Provide medication management including prescription refills and medication education.
• Write prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements.
• Provide in-service staff training and consultation to the social service team.
• Coordinate with Program Director, Assistant Director-Clinical, Program Nurse, and Social Service staff to ensure shelter residents receive ongoing psychiatric treatment in the community.
• Meet with all new residents within the first 30 days for psychiatric evaluation.
• Provide referrals for transitional Psychiatric Treatment to all residents not currently in care.
• Review health assessments completed by the Program Nurse and provide clinical feedback.
• Administer medications including those administered by injection.
• Prepare Psychiatric Assessments for HRA 2010e Housing Application or other applications for housing or needed services.
• Provide training to other team members on topics including medications and drug interactions.
• Communicate and coordinate care with psychiatrists in the community as needed.
• Refer residents to emergency care if needed, coordinate with emergency responders/emergency room staff, and assist with treatment and discharge planning.
• Provide crisis intervention and address all emergency situations as appropriate.
• Assist in building evacuation during fire drills and building emergencies.
• Maintain CPR/First Aid Certification.
• Maintain up-to-date, accurate, complete, and timely consumer chart documentation.
• Attend staff meetings, case conferences, group supervision, and trainings as needed.
• Attend face-to-face supervision with Collaborating Psychiatrist at least monthly.
• Make chart work available to Collaborating Psychiatrist for quarterly reviews.
• Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.

Requirements

• Licensed as a Registered Nurse in New York State and credentialed by the State to practice as an advanced Nurse Practitioner with a specialty in psychiatry.
• Registered as a NY State Medicaid provider.
• DEA# and an individual NPI # required.
• Must carry individual malpractice insurance.
• Preference given to those with experience working on a multi-disciplinary team and/or previous experience with homeless populations.
• Ability to work independently and as part of a team, assisting adult males of diverse ethnicities, backgrounds, ages, and sexual preferences.
• MAT (Medication-Assisted Treatment) experience required.
• Bilingual preferred.

Schedule

A work schedule that includes some weekend and evening hours will be required.
