Responsibilities

• Manage behavioral conditions in collaboration with social workers, psychiatrists, and medical providers
• Perform comprehensive behavioral health assessments
• Document findings consistent with Optum and CMS regulations
• Facilitate agreement and implementation of the member’s plan of care by engaging the care team
• Evaluate the effectiveness, necessity, and efficiency of the plan, making revisions as needed
• Assure the plan aligns with the member's needs and wishes

Requirements

• Current, unrestricted NP license in the state of Rhode Island or ability to obtain prior to start date
• PMHNP Certification
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
• 1+ years of experience in a long-term care setting or community setting
• Valid driver's license with the ability to travel between nursing homes and/or care settings within a designated area

Qualifications

• 3+ years of experience in a long-term care setting or community setting preferred
• Demonstrated post-grad hands-on clinical experience preferred
• Demonstrated ability to move up to 30 pounds preferred
