Psychiatric Mental Health Nurse Practitioner (Full-Time)

About the role

Provide consultative behavioral health services to primary care providers in long-term care environments, including nursing homes and assisted living facilities. Collaborate with care teams to manage behavioral conditions, perform assessments, and develop individualized care plans aligned with regulations. Educate facility staff and families, facilitate care transitions, and evaluate treatment effectiveness. Prioritize patient-centered, non-pharmacological approaches and ensure care plans are cost-effective and responsive to changing needs.

Requirements

• Unrestricted NP license and PMHNP certification
• Experience in long-term or community care settings
• Ability to travel between care sites
• Strong communication, assessment, and care coordination skills
• Creativity in behavioral health approaches in LTC settings
