About the role
Cabana is a behavioral health infrastructure company dedicated to eliminating the gap between self-guided wellness and clinical intervention. We combine physical Cabana Pods, a daily digital support platform (Cabana Live), and an integrated Collaborative Care (CoCM) model to deliver a zero-friction Mental Health Operating System for high-performing organizations.
Responsibilities
- Provide systematic psychiatric consultation to Behavioral Health Care Managers (BHCMs) through Structured Case Review (SCR), with emphasis on: High-risk patients, Patients new to treatment, Individuals not meeting clinical targets, Complex diagnostic presentations
- Advises Primary Care Providers (PCPs) regarding: Evidence-based pharmacologic treatment recommendations, Diagnostic clarification strategies, Lab monitoring and medication management considerations
- Document treatment recommendations directly within partner electronic health record systems to support seamless care coordination
- Remain available for occasional urgent consultations with PCPs regarding complex patient presentations, targeting response times within one business day (or one hour for urgent needs)
- Utilize Cabana’s clinical registry and measurement-based care systems to: Track patient progress across assigned caseloads, Identify patients requiring treatment escalation or care adjustments, Support population-level behavioral health management initiatives
- Review validated outcomes data utilizing tools such as: PHQ-9, GAD-7, Columbia Suicide Severity Rating Scale (C-SSRS) Support treatment-to-target care planning and symptom remission strategies
- Serve as a clinical mentor and psychiatric resource for PCPs and BHCMs Provide guidance regarding: Behavioral health treatment planning, Diagnostic conceptualization, Medication management strategies, Management of complex psychiatric presentations
- Collaborate closely with interdisciplinary care teams to improve integration of behavioral health services within primary care settings Maintain strong communication and collaborative relationships across remote care teams
- Partner with Cabana’s Clinical Director to: Design and refine psychiatric consultation workflows, Develop escalation pathways and consultation protocols, Establish documentation standards for CoCM implementation
- Support development of: Standard operating procedures (SOPs), Clinical workflow frameworks, Quality assurance processes, Pilot program launch strategies
- Participate in quality review meetings and contribute psychiatric insight to operational and performance improvement discussions
- Perform all other duties, as assigned.
Requirements
- MD or DO with an active, unrestricted Louisiana medical license Board Certified in Psychiatry (ABPN or equivalent)
- Minimum of 3+ years of clinical experience, preferably in one or more of the following: Integrated behavioral health, Primary care behavioral health, Consultation-liaison psychiatry, Collaborative Care (CoCM) environments
- Strong knowledge of evidence-based psychiatric medication management for common behavioral health conditions, including: Anxiety disorders, Depression, PTSD, Behavioral health conditions with medical comorbidities
- Experience utilizing measurement-based care tools, including: PHQ-9, GAD-7, C-SSRS
- Experience working within electronic health record (EHR) systems, such as Epic, Cerner, Other equivalent EHR platforms
- Strong written communication and clinical documentation skills
- Must reside within the United States while maintaining active Louisiana medical licensure
- Strongly Preferred: Prior experience working within the Collaborative Care Model (CoCM), Familiarity with “treat-to-target” protocols and registry-based population health management, Experience consulting within: Primary care settings, Federally Qualified Health Centers (FQHCs), Integrated care environments, Familiarity with CoCM and Behavioral Health Integration billing requirements, including CPT codes: 99492, 99493, 99484, Comfort managing population-based caseloads rather than traditional appointment-only psychiatric models, Strong collaboration and interdisciplinary communication skills, Demonstrated ability to build workflows and contribute to clinical program development initiatives, High level of adaptability and comfort operating in early-stage program environments
