About the role

Community Bridges, Inc. (CBI) is seeking a PMHNP to join our team. The position is based in the onsite clinic in Mesa, AZ, with a flexible schedule of Mon/Wed/Friday 8AM-5PM.

Responsibilities

  • Completes Urgent Psychiatric and/or Addiction Medicine evaluations with all patients referred to the 23-hour observation unit from various referral sources.
  • Provides patient-centered, evidence-based care for patients, serving as a patient advocate in all health-related matters.
  • Coordinates with other healthcare providers to ensure a smooth transition for patients moving between different CBI programs or levels of care.
  • Assesses and writes orders for seclusion and restraint interventions, ensuring they are documented and authorized within the specified limits.
  • Reviews and documents discharge plans within 24 hours of discharge, ensuring follow-up recommendations are included.
  • Participates in quality management initiatives and corrects identified deficiencies within 72 hours.
  • Maintains a minimum 85% patient satisfaction rate and a minimum 75% follow-up rate.
  • Utilizes e-Prescribing for all patient prescriptions and attends regular staffing and rounds on assigned patients.
  • Supports utilization management activities and promotes collaboration among healthcare professionals.

Qualifications

  • Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP)
  • Experience in inpatient and outpatient settings
  • Knowledge of CBI's policies and procedures
  • Ability to communicate effectively with patients, families, and other healthcare providers
  • Proficiency in e-Prescribing and documentation systems
