Program Therapist

About the Role

Salt Lake Behavioral Health is seeking a Full-Time Program Therapist (Social Worker/(A)CMHC) for various units. This position will work collaboratively with the treatment team, families, community resources, and other healthcare providers as part of the social services team.

Responsibilities

• Prioritize daily tasks including new admissions and discharging patients along with seeing patients on given caseload.
• Participate daily in a multi-disciplinary treatment team meeting.
• Conduct psychosocial assessments, ASAM’s, and health disparities forms for new admissions.
• Educate, communicate, and coordinate discharge plans with patients' families and support systems.
• Provide daily individual therapy along with psycho-educational groups.
• Complete discharge planning including crisis safety plans, family meetings, and discharge summaries.
• Complete documentation in a timely manner according to the given standards of the hospital.

Qualifications

• Master's Degree from an accredited college or university in Social Work or a clinical-related mental health field required.
• License: CSW, LCSW, ACMHC, CMHC, AMFT, or LMFT with the state of Utah.
• CPR certification is required.
• Successful completion of training for restraints and seclusions within 90 days of employment.

Pay

Pay Range CSW/ACMHC (Exempt): $32.30-$42.80/hr

Pay Range LCSW/CMHC (Exempt): $36.30-$46.80/hr
