Problem Solving Specialist

Responsibilities

  • Receive, review, and manage referrals from the Family Solutions Center (FSC).
  • Conduct assessments to identify housing barriers, strengths, resources, and immediate needs.
  • Provide problem-solving interventions for families experiencing homelessness or at risk of homelessness.
  • Facilitate mediation and conflict resolution between clients, family members, and friends that can support keep the family stabilized.
  • Connect participants to community resources, mainstream benefits, emergency assistance programs, and supportive services.
  • Maintain accurate and timely documentation of client interactions, assessments, referrals, case notes and outcomes in applicable databases and case management systems.
  • Monitor participant progress and provide ongoing follow-up to ensure successful resolution of housing crises.
  • Track performance measures and program outcomes in accordance with agency and funder requirements.
  • Participate in case conferencing and multidisciplinary team meetings to support coordinated care.
  • Advocate on behalf of clients to reduce barriers to housing and services.
  • Maintain knowledge of local housing resources, landlord engagement strategies, and homelessness prevention programs.
  • Aid in outreach and engagement efforts to identify and support households in housing crisis.
  • Perform other related duties as assigned.
  • Collect and report program data in compliance with agency policies, grant requirements, and funder expectations.
  • Participate in case reviews, staff meetings, trainings, and multidisciplinary case conferencing.
  • Utilize trauma-informed, strengths-based, and housing-first practices when working with participants.
  • Perform additional tasks, projects, and responsibilities as assigned by the supervisor

Qualifications

  • Minimum of two (2) years of experience providing case management, housing navigation, homelessness prevention, crisis intervention, or related social services.
  • Experience working with individuals and families experiencing homelessness or housing instability.
  • Experience conducting client assessments, service planning, and resource coordination.
  • Experience working with diverse and vulnerable populations using a trauma-informed and strengths-based approach.

Skills

  • Strong problem-solving, crisis intervention, and conflict resolution skills.
  • Knowledge of homelessness services, housing navigation, and community resources.
  • Ability to build rapport with diverse populations and work effectively with community partners.
  • Excellent organizational, communication, and documentation skills.
