Responsibilities

• Provide therapy services as needed in accordance with patient needs and treatment plans.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Document patient interactions and progress in electronic health records.
• Adhere to all regulatory requirements and maintain confidentiality of patient information.

Requirements

• Current Georgia License.
• At least one year of experience working in a Long-term Care/Skilled Nursing community.
• Experience working in a Person-Directed Care community preferred.
• CPR/BLS First Aid Certification.
• Genuine passion for working with elders.
• Ability to work at least one shift per month.
• Strong team work/collaboration skills.
• Excellent customer service and communication skills.
• Strong organization and time management skills.
