Responsibilities

• Meets new and prospective clients and performs an initial assessment of service needs – this could be done virtual and/or in-person, depending on the client's request.
• Develops a Plan of Care for all new clients and updates as needed.
• Communicates with care managers, caregivers, office staff, clients, family members, and other medical professionals about client's needs and concerns.
• Consults with the Clinical Director regarding the supervision of services.
• Provides training to the clients regarding their care, when requested.
• Assures proper maintenance of clinical records and documentation in compliance with State and Federal laws.
• Performs client evaluations every 30, 90, or 120 days as needed by the client's Plan of Care.
• Performs caregiver evaluations every 30, 90, or 120 days as needed by state requirements.
• Provides skills evaluations for new staff during the orientation process.
• Trains new staff and existing staff.
• Provides Clinical Oversight.

Qualifications

• RN State License – MD with Trach care experience is required.
• Current CPR and First Aid certification.
• TB vaccination and Hep B vaccination.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Clear criminal background and driving record.
• Two professional references within the healthcare field.
• Prior home care experience is strongly preferred.
• Self-starter with the ability to work independently and manage time effectively.
• Strong interpersonal skills, including a willingness to teach and mentor caregivers.
• Comfort with technology; prior experience with electronic medical records preferred.
• Ability to drive and travel to cases as assigned.
