PRN Recovery Specialist

Summary Of Position

Provides support services, provides information, education and support for clients, promotes wellness, recovery and skill building to sustain better health.

Promotes prevention training for managing stress and problems, making healthy lifestyle choices, recognizing and responding to relapse, managing crisis, and achieving and sustaining recovery.

Aids clients in developing personal recovery goals, building natural support systems, and coaching clients to advocate for themselves within their healthcare systems.

Essential Duties & Responsibilities

  • Provides group or individual training/interventions focusing on improvement of the client’s functioning and health-related issues.
  • Aids clients in developing their Wellness Recovery Action Plan (WRAP).
  • Offers wellness consultation.
  • Works in collaboration with clients, family members, and other providers to implement the plan.
  • Empowers clients to self-manage chronic health conditions and solidify wellness goals.
  • Aids clients in overcoming barriers to goals, including access to care.
  • Supports clients in navigating health care systems.
  • Links clients to community-based social support/services and develops collaborative relationships with other departments, services, and community health care agencies to facilitate and support wellness and quality care.
  • Provides ongoing case management services for assigned service recipients, including assessment, planning, linking, monitoring, referral, and advocacy.
  • Assists clients in achieving and maintaining wellness goals through transportation assistance.
  • Conducts intake for clients entering services, explains services, gathers necessary information, ensures signatures on appropriate contracts and consents, and submits materials for processing.
  • Conducts assessments and writes assessment summaries, as assigned.
  • Aids clients in creating discharge plans and provides follow-up services as assigned.

Knowledge, Skills & Abilities

  • Effectively communicates via written, verbal, in-person, and virtual methods.
  • Knowledge of role and function of various community resources.
  • Knowledge of and ability to instruct assigned clients in basic independent living skills.
  • Skill in customer service/experience techniques.
  • Skill in problem-solving.

Qualifications

  • Minimum of 21 years of age per funding requirements.
  • Associates degree required. Bachelor’s degree in human services preferred.
  • Education/experience equivalency may be considered upon approval.
  • Two years relevant experience working in the human services field.

Required Certification/Licensure

WRAP facilitator with in twelve (12) months of accepting position is preferred.
