About the role
Join Newport Meadows Nursing and Rehabilitation Center in Christiana, Pennsylvania as a PRN Physical Therapist (PT). In this position you will provide high‑quality physical therapy services to a diverse patient population, including assessment, treatment, and education. You will collaborate closely with an interdisciplinary team to develop and implement individualized care plans that meet each patient’s unique needs, while maintaining accurate records and communicating with physicians and other healthcare professionals.
Responsibilities
- Conduct thorough patient assessments to identify physical therapy needs and develop individualized care plans.
- Implement and conduct physical therapy treatments, including modalities, exercises, and other interventions.
- Monitor and document patient progress, adjusting treatment plans as needed.
- Collaborate with the multidisciplinary team, including physicians, occupational therapists, speech therapists, and other healthcare professionals.
- Maintain accurate and up‑to‑date patient records, including progress notes and treatment plans.
- Participate in patient care conferences, discharge planning, and other interdisciplinary meetings.
Requirements
- Current licensure as a Physical Therapist in the state of Pennsylvania.
- Minimum of 0 years of experience in acute care, rehabilitation, or a related setting.
- Strong communication and interpersonal skills.
