PRN Physical Therapist (PT)

Enhance Rehabilitation is currently seeking qualified Physical Therapist candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be part of a growing team providing the highest quality care for patients in a skilled‑nursing‑facility (SNF) setting, evaluating and treating patients according to individualized plans of care.

Responsibilities

• Evaluate and treat patients following an individualized plan of care.
• Provide excellent care and customer service to patients and their family members.
• Collaborate with facility staff to ensure coordinated, high‑quality patient care.
• Evaluate new patients, develop plans of care, and set measurable, functional goals.
• Treat patients, address their concerns, and educate them on injuries, progress, and probable outcomes.
• Supervise Physical Therapist Assistants.
• Re‑evaluate patient progress, revise plans of care, and reset goals as needed.
• Complete timely and accurate documentation, including evaluations, re‑evaluations, discharges, and daily progress notes.
