PRN Physical Therapist

About the role

Deliver high-quality physical-therapy services in patients’ homes, focusing on mobility, pain reduction, and functional restoration. Work autonomously while collaborating with physicians and the broader care team to ensure equitable, patient-centered outcomes.

Responsibilities

• Conduct comprehensive patient assessments in home settings.
• Develop and implement individualized treatment plans.
• Monitor patient responses, document progress, and adjust care as needed.
• Educate patients and families on treatment plans and home-exercise programs.
• Collaborate with physicians and healthcare team members.
• Adhere to state practice laws and maintain quality standards.

Requirements

• Current state licensure as a Physical Therapist.
• Valid driver’s license, vehicle insurance, and reliable transportation.
• CPR certification (preferred or must be obtained within 90 days of hire).
