About the role
Deliver high-quality physical-therapy services in patients’ homes, focusing on mobility, pain reduction, and functional restoration. Work autonomously while collaborating with physicians and the broader care team to ensure equitable, patient-centered outcomes.
Responsibilities
- Conduct comprehensive patient assessments in home settings.
- Develop and implement individualized treatment plans.
- Monitor patient responses, document progress, and adjust care as needed.
- Educate patients and families on treatment plans and home-exercise programs.
- Collaborate with physicians and healthcare team members.
- Adhere to state practice laws and maintain quality standards.
Requirements
- Current state licensure as a Physical Therapist.
- Valid driver’s license, vehicle insurance, and reliable transportation.
- CPR certification (preferred or must be obtained within 90 days of hire).
