About the role
Sunny Hill Nursing Home in Joliet, IL, invites you to join our dynamic therapy team as a PRN Physical Therapist. Weekend availability preferred.
Responsibilities
- Develop and implement individualized treatment plans based on assessment findings
- Provide therapeutic exercise, modalities, and patient education to promote functional independence
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Evaluate patient progress and adjust treatment plans as necessary
- Document all treatments and progress notes accurately and timely
Requirements
- Current state licensure as a Physical Therapist
- At least one year of clinical experience in a healthcare setting
- Bachelor's degree in Physical Therapy from an accredited program
- Passing scores on the National Physical Therapy Examination (NPTE)
Qualifications
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficiency in using electronic health records (EHR) systems
