PRN Physical Therapist

About the role

Enhance Rehabilitation is seeking qualified Physical Therapist candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be part of a growing team providing the highest quality care for patients in a Skilled Nursing Facility (SNF) setting. In this role you will evaluate and treat patients following an individualized plan of care.

Responsibilities

• Provide excellent care for patients and deliver high‑quality customer service to patients and family members.
• Collaborate with facility staff to ensure excellent care for patients.
• Evaluate new patients, develop plans of care, and set measurable, functional goals.
• Treat patients, address their concerns, and educate them on their injury, progress, and probable outcomes.
• Supervise Physical Therapist Assistants.
• Re‑evaluate patient progress, revise plans of care, and reset goals.
• Complete timely and accurate documentation, including evaluations, re‑evaluations, discharges, and daily progress notes.

Qualifications

• Current licensure as a Physical Therapist.
• Experience or willingness to work in a Skilled Nursing Facility (SNF) setting.
• Ability to develop individualized plans of care and set functional goals.
• Strong communication skills for patient education and collaboration with staff.
• Experience supervising Physical Therapist Assistants is preferred.
