PRN Physical Therapist

About the role

Enhance Rehabilitation is currently seeking qualified Physical Therapists candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be a part of a growing team providing the highest quality care for patients in a SNF setting.

Responsibilities

• Evaluate and treat patients following an individualized plan of care.
• Provide excellent care for patients.
• Provide excellent customer service to patients and family members.
• Collaborate with facility staff to ensure excellent care for patients.
• Evaluate new patients, develop plans of care and set measurable, functional goals.
• Treat patients, answer their concerns and educate them on their injury, progress and probable outcomes.
• Supervise Physical Therapist Assistants.
• Re‑evaluate patient progress, revise plans of care and reset goals.
• Complete timely and accurate documentation including evaluations, re‑evaluations, discharges and daily progress notes.
