About the role
Conduct telephonic and in-person assessments to determine program eligibility
Coordinate wraparound services for individuals with complex medical, behavioral, or social needs
Educate clients and caregivers on available resources and care planning
Advocate for clients facing health and safety risks or potential institutionalization
Collaborate with internal teams, healthcare providers, and community partners
Responsibilities
- Conduct telephonic and in-person assessments to determine program eligibility
- Coordinate wraparound services for individuals with complex medical, behavioral, or social needs
- Educate clients and caregivers on available resources and care planning
- Advocate for clients facing health and safety risks or potential institutionalization
- Collaborate with internal teams, healthcare providers, and community partners
Requirements
- Active Ohio LSW or RN license in good standing
- At least one year of experience in medical social work, home health, or geriatrics
- Strong understanding of aging-related issues and community resources
Qualifications
- High-speed home internet for accessing agency systems
- Valid driver’s license and reliable transportation
- Excellent communication, empathy, and clinical judgment
- Above-average technology skills and adaptability
