PRN Nurse Practitioner,Homevisits/Telehealth - Per Visit

Responsibilities

• Covers comprehensive in-home health risk assessments to identify active and chronic disease conditions, and assesses physical, mental, and social needs.
• Takes patient history, performs examinations, determines diagnoses, and documents visits according to NCQA standards.
• Takes patient vital signs as needed, places case management referrals, and communicates with primary care physicians as necessary.
• Communicates with patients, caregivers, agency nurses, other providers, and vendors to ensure proper diagnosis and treatment.
• Maintains patient confidentiality and observes OSHA Universal Precautions.
• Attends required meetings, in-services, and participates in committees as requested.
• Participates in professional development activities and maintains professional licenses and affiliations.

Qualifications

• Active/unrestricted nurse practitioner license in coverage states.
• Board certification in one of the following: ANCC, AANP, or NCCPA.
• Active BLS Certification.
• Current enrollment in Medicare/Medicaid.
• Maintain a valid driver’s license and good driving record.
• Outstanding EHR skills.

Preferred Qualifications

• Geriatric training/experience.
• Teamwork and effective communication skills with patients, medical staff, and the public.
