About the role

The Licensed Vocational Nurse (LVN) provides skilled, patient-centered care in patients' homes, supporting safe condition management and promoting independence and quality of life.

Responsibilities

• Perform nursing visits
• Administer medications and treatments within scope
• Monitor patient conditions and health
• Report changes in patient status
• Educate patients and caregivers on medications and safety
• Maintain accurate documentation
• Follow infection‑control procedures
• Travel within assigned areas
• Collaborate with the care team
• Follow physician care plans
• Adhere to policies and regulations

Qualifications

• Active, unrestricted Texas LVN license
• Current CPR/BLS certification
• Reliable transportation
• Valid driver’s license
• Ability to work independently
• Preferred: at least one year of experience in nursing, home health, hospice, or palliative care
