About the Role

Cherish Hospice is seeking a dedicated PRN Social Worker to join our compassionate team in Youngstown, Ohio. This vital role provides flexible, as-needed psychosocial support to patients, families, and caregivers during one of life's most profound transitions.

Responsibilities

  • Conduct psychosocial assessments within 5 days of admission, identifying emotional, social, financial, and environmental needs.
  • Participate in IDG meetings and collaborate with the interdisciplinary team to deliver whole-person care.
  • Observe, record, and report changes in the patient's emotional and social well-being throughout the course of care.
  • Assess the patient's and family's ability to cope with the dying process, offering meaningful support every step of the way.
  • Evaluate the effectiveness of social work interventions and adjust approaches to meet evolving needs.
  • Participate in discharge planning as needed.

Requirements

  • Master's in Social Work (MSW), licensed in the State of Ohio
  • Minimum 1 year of experience, preferably in hospice or palliative care
  • Strong interpersonal, communication, and crisis intervention skills
  • Cultural sensitivity and respect for diverse backgrounds
  • Deep commitment to compassionate, patient- and family-centered care
