Role: PRN Nurse Practitioner providing hospice care in the Waco area and surrounding counties. Responsibilities: Assess patient eligibility, evaluate physical and psychosocial needs, coordinate with physicians and interdisciplinary teams, and conduct regular home visits to reassess patients and update care plans. Qualifications & Skills: Nursing license with Nurse Practitioner certification, strong assessment and communication skills, ability to collaborate with healthcare teams, and dedication to patient-centered care.
