PRN Home Infusion Nurse

About the Role

An Infusion Nurse I is a licensed registered nurse who provides basic direct patient care to patients in the home or alternate infusion site, ensuring patient safety with continuity and compliance under a physician’s plan of care. This role operates under very close direct supervision.

Responsibilities

• Initiates, develops, and implements a basic nursing plan of care, evaluating patient progress toward goals.
• Provides direct patient care, performs treatments, administers medications, and educates patients and families/caregivers.
• Modifies the plan of treatment in response to changing patient status based on physician orders to achieve established or revised patient care goals.
• Assesses patient needs and physical status at each visit through health data access and patient interviews; re-evaluates needs through reassessment and response to therapy.
• Obtains and clarifies physician orders for plan of treatment revisions; informs physicians promptly of significant changes in patient condition and provides written summaries.
• Delivers effective, patient-centered teaching to achieve independence with prescribed therapy and care needs.
• Communicates timely and effectively with Naven Health’s Clinical team, medical providers, patients, and families to facilitate continuity of care.
• Prepares real-time clinical documentation during visits and submits it to the nursing department to comply with billing timelines.
• Completes all documentation legibly, using approved abbreviations and error-correction practices per Naven Health’s policy.
• Coordinates discharge planning, prepares discharge summaries, and thoroughly reports patient care needs when transferring care.
• Demonstrates compliance with agency operations, policies, procedures, professional standards, and regulatory/accreditation requirements.
• Maximizes work efficiency using computers and automation technologies to validate orders and communicate care.
• Responds promptly to patient requests and initiates emergency procedures as necessary.
• Accepts accountability for ethical and professional conduct in practice.
• Follows established programs and practices aligned with Naven Health’s mission, values, and objectives.
• Safeguards patient and proprietary information confidentiality, adhering to HIPAA regulations.
• Educates patients about Naven Health’s scope of services and related care plans.
• Completes required training, in-services, and continuing education to maintain licensure and competency.
• Provides oversight and coordination of paraprofessionals in the home setting; may act as Supervisor in the absence of the Nurse Manager per state/federal regulations.
• Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses.
• Participates in nursing on-call responsibilities as applicable.
• Performs other related duties as directed by supervisor.

Requirements

• Active and unrestricted RN license in the state of practice.
• Minimum of 0-2 years of infusion nursing experience.
• Current CPR certification (in-person class required per company policy).
• Demonstrated competency in patient care standards for safe service delivery and infusion skill sets.
• Required licensure to operate a motor vehicle in the state of practice, with access to a vehicle for business travel and proof of liability insurance.
• Basic knowledge of computer operating systems and software applications for effective use of nursing technology tools.

Qualifications

• Ability to lift up to 50 pounds (maximum lifting of 75 pounds).
• Exerting up to 20 pounds of force occasionally or up to 10 pounds frequently.
• Physical demands include walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting, and carrying.
• Fine motor skills and visual acuity, including close and distance vision, color and peripheral vision, depth perception, and ability to adjust focus.
• Likelihood of occupational exposure to blood, body fluids, and other potentially infectious materials.
• Possible exposure to hazardous substances, adverse environmental conditions, animals, and unpredictable home settings.
• Safety requirements include closed-toe shoes, facemask, goggles, gown, gloves, and a functioning communication device.
• Willingness to travel 100% of the time for business purposes.

Preferred Qualifications

• Bachelor of Science in Nursing from an accredited school of nursing.
• 1 year of previous RN experience in a home health or acute care setting.

Pay

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment. Pay range is $38.84–$64.72 per hour.
