Key Responsibilities
- Provide skilled nursing care and assessments to patients in their homes on an as-needed basis
- Develop and implement individualized care plans according to physician orders
- Collaborate with interdisciplinary teams and communicate effectively with patients and their families
- Maintain accurate and timely documentation of patient care
- Ensure compliance with health, safety, and regulatory standards
Requirements
- Current Michigan Registered Nurse (RN) License
- Minimum one year of home health nursing experience preferred
- Valid driver's license and dependable transportation
- Excellent clinical assessment, organizational, and communication skills
- Able to work independently and adapt to varying patient needs
- CPR certification required
- Must pass background check and drug screening
