About the role
As a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you’ll play a vital role in delivering compassionate, personalized care to patients in the comfort of their homes. In collaboration with the Registered Nurse and care team, you’ll help manage medical needs and promote their independence — caring for patients like family, one home visit at a time.
Healthcare at Home is a Medicare-certified service line from HCA Healthcare, dedicated to providing patients with quality care during their recovery at home. Our patients are under the supervision of a physician, and our team of professionals develops an individualized plan of care. Our team includes registered nurses, licensed practical nurses, therapists, and social workers.
Responsibilities
- Provide nursing care in the home, including wound care, medication administration, and support during procedures
- Document assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care
- Monitor patient status and alert the care team of any changes to ensure timely, coordinated responses
- Educate patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home
- Collaborate closely with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized care in the home setting
Requirements
- Current Licensed Practical or Vocational Nurse (LPN/LVN) license
- Valid Driver License (DL)
- Graduate of a vocational school
- Basic Cardiac Life Support certification (must be obtained within 30 days of employment start date)
- 1 year of experience as an LPN/LVN
