PRN Home Care CNA

Responsibilities

• Provide direct in-home care for seniors and those with an illness, injury or chronic disease
• Help with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake
• Auxiliary tasks including light housekeeping, meal prep, medication reminders, and transportation
• Ambulate, assist with transfers and range of motion exercises
• Monitor vitals, measure intake/output, check blood glucose and document changes
• Collaborate with a team of home health nurses and aides to follow the care plan
• Observe patients and report any issues requiring changes in care, medication or medical equipment
• Perform other CNA/PCA duties as assigned, in accordance with the care plan and supervision guidelines

Caregiver Qualifications

• Current CNA certification
• At least 6 months of previous experience in home health or long-term care setting preferred
• Current CPR certification
• Negative TB test (within past 12 months)
• Valid driver’s license, auto insurance and transportation
• Pass pre-employment drug screen and background check
• Compassionate nature, dependability, good communication and ability to lift up to 50 lbs.
