What You’ll Do

As the Licensed Master Social Worker, you will conduct psychosocial evaluations, develop knowledge of the needs for clinically complex dialysis patients, and utilize your patient advocacy skills to drive positive outcomes.

Minimum Qualifications

  • Master’s degree from an accredited graduate school of social work
  • Minimum of 12 months professional work experience as a MSW in a dialysis setting
  • State licensure, if required or available in the state where facility is located

Essential Duties and Responsibilities

  • Assesses patients' psychosocial status, strengths, and areas of need affecting rehabilitation and optimal treatment outcomes
  • Collaborates with the patient and healthcare team to develop care plans aimed at improving treatment adherence, vocational/educational rehabilitation, and quality of life
  • Provides psycho-educational counseling and supportive counseling to patients seeking transplant
  • Affords support and goal-directed counseling to patients regarding advance directives and Do Not Resuscitate (DNR) orders
  • Assists patients in accessing community resources for vocational rehabilitation and education
  • Participates in the transplant referral process and collaborates with the interdisciplinary team
  • Reviews patient rights and responsibilities, grievance information, and facility policies
  • Addresses patient issues related to insurance and provides information about payment options
  • Reassesses patient demographic and insurance information changes quarterly and notifies relevant departments
  • Maintains current knowledge of local vocational/educational rehabilitation programs and assists patients with referrals
  • Collaborates with the interdisciplinary team to provide information about treatment modalities and facilitate the referral process
  • Provides information and education to patients about payment to dialysis, including federal, state, commercial insurance, state renal programs, AKF HIPP, and entitlement programs
  • Assists in building and maintaining a facility/clinic culture of safety and quality
  • Reports errors and near-misses to prevent recurrence and addresses negative feedback, complaints, and grievances from staff, patients, and families
  • Participates in performance improvement projects and attends scheduled meetings
  • Participates in annual QAPI training and other continuing education as assigned and required
  • Provides suggestions for alternate strategies and identifies opportunities for improvement
  • Assists in the development and maintenance of the facility/clinic’s EHR system
  • Ensures continuity of care as patients transition between facilities
  • Displays a proactive approach, assertsiveness, and a positive attitude in all aspects of governing unit functions
  • Uses evidence-based guidelines, proper infection control practices, and displays a proactive approach and assertiveness in all aspects of governing unit functions
  • Maintains patient-specific psychosocial information in the Rendevor Dialysis EHR system
  • Works with the interdisciplinary team to establish, document, and update patients’ plan of care
  • Communicates with staff, partners, vendors, and medical directors via email at least weekly
  • Adheres to company policies and procedures and monitors staff compliance
  • Performs other duties as assigned

Supervision

This position reports directly to the Clinical Director, Residential Dialysis.

Work Environment

This job exists in a hospital, training center, patient residence, or in the medical unit of a post-acute or correctional facility. This role may include exposure to chemicals and blood-borne pathogens. This role routinely involves the use of both medical and standard office equipment.

Physical Demands

  • Sit, stand, walk, use hands to handle or feel, and reach with hands and arms
  • Push, pull, or lift supplies and products up to 50 pounds occasionally
  • Close vision and ability to adjust focus
