PRN Case Manager (RN)

About the role

We're seeking a PRN Case Manager RN to join our Case Management team at Valley View Hospital. The position involves assisting inpatient and outpatient patients and their families with their care plans across the healthcare continuum.

Responsibilities

• Affords support to patients and their families in managing their care plans.
• Collaborates with interdisciplinary teams to ensure comprehensive patient care.
• Coordinates discharge planning services for inpatients and outpatients.
• Utilizes computer skills, particularly word processing and spreadsheet software, and familiarity with Epic and Meditech systems.

Requirements

• Bachelor of Science in Nursing (BSN) preferred.
• Graduated from an accredited or state board of nursing approved Registered/Professional Nursing program.
• Current state licensure as a Registered Nurse (RN).
• One year of nursing (RN) experience.
• Previous case management experience preferred.

Qualifications

• Education: BSN RN preferred.
• Experience: Case management experience preferred.
• Computer experience: Word processing, spreadsheet, and familiarity with Meditech systems.

• Excellent communication and interpersonal skills.
• Strong organizational and problem-solving abilities.
• Ability to work independently and as part of a team.

Pay and Schedule

• Salary range: $39.96 to $59.95/hour +14% PRN Differential Pay.
• Entry salary dependent upon education, skill set, and experience.

Schedule

• PRN (Per Request Notional) schedule available.
