About the role

The Registered Nurse (RN) at Suncrest Hospice is responsible for providing comprehensive care and services to a group of patients, including those residing in private homes, Skilled Nursing Facilities, and assisted living facilities. This role contributes to the interdisciplinary team, offering expertise in psychosocial assessments, interventions, and knowledge of community resources.

Responsibilities

  • Provide care and services to patients in various settings such as private homes, Skilled Nursing Facilities, and assisted living facilities.
  • Collaborate with the interdisciplinary team, including social workers, chaplains, volunteers, and Certified Nursing Assistants (CNAs).
  • Work independently and on a 1-1 level with patients, supported by a patient care team.
  • Manage time effectively to balance visits, documentation, and participation in meetings.
  • Use an electronic medical records system to manage patient care.
  • Drive with reliable transportation, valid driver's license, and auto insurance to cover the service area.

Requirements

  • Current Registered Nurse (RN) licensure in the state(s) of practice.
  • Works well within a team environment, collaborating daily with various healthcare professionals.
  • Strong time management skills to prioritize and address multiple concerns.
  • Willingness to learn and utilize an electronic medical records system.

Qualifications

  • Commitment to providing high-quality care to patients and their families.
  • Experience working in a hospice setting preferred, but not required.

Skills

  • Excellent communication and interpersonal skills.
  • Knowledge of hospice policies and regulations.
  • Ability to handle sensitive and confidential information.
