Responsibilities

• Deliver exceptional care to clients in their homes
• Execute comprehensive personalized weekly schedules and care plans
• Provide skilled nursing services as prescribed under the client’s plan of care, within scope of practice
• Counsel clients and families to meet nursing and related needs
• Complete all documentation requirements per agency policy and procedure
• Coordinate services by participating in case conferences, case management, team meetings, and staff meetings as indicated
• Perform administrative functions such as documenting task completion and reporting changes in the client’s physical condition, mental capability, or behavior

Qualifications

• Licensed Practical Nurse (LPN) license
• Basic Life Support (BLS) certification
• Experience placing peripheral IVs, accessing ports, and working with PICC lines
• Ability to use sound judgment and remain calm in a variety of situations
• Basic computer skills
• Proof of eligibility to work in the U.S.
• Background screening through the Florida Clearinghouse
