Responsibilities

  • Licensed Practical Nurse (LPN) provides direct patient care as defined in the State Nurse Practice Act.
  • LPN implements a plan of care initiated by the clinician.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Licensed Practical Nurse (LPN) assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • LPN participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the clinician and/or Clinical Supervisor.
  • LPN uses equipment and supplies effectively and efficiently.
  • Licensed Practical Nurse (LPN) participates in personal and professional growth and development.
  • Performs other duties as assigned by the Registered nurse.

Requirements

  • Currently licensed as a Licensed Practical Nurse in Pennsylvania.
  • Valid CPR Certificate.
  • At least two years experience working with pediatric patients.
  • Compliant with accepted professional standards and practices.
  • Demonstrates good verbal and written communication, and organizational skills.
