Responsibilities
- Provide clinical services to clients struggling with coping skills and emotional regulation.
- Facilitate 4 groups on a weekly basis and meet with clients on a weekly basis.
- Make biweekly contact with family or support members.
- Assess counseling needs and diagnose substance abuse and mental health conditions using DSM, PHQ-9, and Columbia Scale criteria.
- Prepare and provide the substance abuse counseling and co-occurring portions of the client treatment plan.
- Develop client discharge/continuum of care plans to ensure movement to appropriate levels of care.
- Document all client records, counseling, education services, assessments, reassessments, referrals, and follow-ups in the client’s clinical record.
- Coordinate support services for the client/family and document the services provided in the client’s clinical record.
- Contact and coordinate with referral sources, including legal, school, employer, etc.
- Facilitate interventions for crisis-oriented issues using a multi-disciplinary team approach.
- Facilitate group sessions and document: discharge, individual sessions, weekly calls, level of care changes, and treatment plans.
Additional Duties
- Absorb and cover other staff when necessary to maintain high productivity and efficiency in the department.
- Assist with additional responsibilities as the company grows and assigns accordingly.
