Primary Nurse RN

Position Summary
The Primary Nurse - RN is responsible for overseeing, assessing, planning, and implementing nursing care for participants in the Program for All-Inclusive Care for the Elderly (PACE) program. This includes collecting health/symptom data, analyzing and interpreting data, planning and implementing nursing interventions, dispensing medication, providing health teaching, evaluating outcomes, and collaborating with other members of the interdisciplinary team.
Responsibilities
- Collect health/symptom data from participants and/or caregivers
- Analyze and interpret recorded data to determine appropriate responses
- Plan and implement nursing interventions, including reviewing documentation and communicating with medical providers
- Dispense medication as ordered by medical providers
- Provide and coordinate health teaching to maintain optimal health levels
- Evaluate outcomes of nursing interventions and initiate changes as necessary
- Communicate with other members of the interdisciplinary team through documentation and meetings
- Serve as a patient advocate, integrating patient education and facilitating care plan modifications
Qualifications
- Current RN license in Massachusetts
- Experience in PACE or similar programs preferred
- Strong clinical skills and ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Knowledge of PACE program requirements and regulations

This position is full-time and works at multiple locations including 10 Garofalo Street - Revere.
