Primary Care Providers (MD/DO, NP or PA) - Central CT Locations

About the role

Community Health Center is seeking a Primary Care Provider to join our team. The ideal candidate will provide a full scope of medical services primarily to underserved populations, receive strong clinical support, and work in an innovative practice environment utilizing Advanced Access scheduling.

Responsibilities

• Provide comprehensive medical services to underserved populations
• Work in an innovative practice environment with advanced scheduling tools
• Participate in a multi-disciplinary team offering both individual and population-based care

Qualifications

• MD/DO from an approved medical/osteopathic school with completion of an accredited residency program in family practice
• Board-certified within 24 months of employment at CHC and hold a valid license in the State of CT
• ANA certification as a Family Nurse Practitioner required
• Graduate of an accredited Master of Science in Nursing program with a focus on Family Nurse Practitioner
• At least 12 months of outpatient experience
• Bi-lingual skills in English and Spanish preferred
