About the role

We are seeking an experienced Primary Care Provider - Advanced Practice Clinician (APC) to independently manage a patient panel in a collaborative, value-based primary care setting.

Responsibilities

• Provide comprehensive, longitudinal, relationship-based care for primary care patients
• Perform assessments, diagnoses, and treatment plans for acute and chronic conditions
• Manage test results, medication, and referrals through inbox responsibilities
• Coordinate care with interdisciplinary team members such as RNs, pharmacists, and specialists
• Partner with patients and families for shared decision-making
• Support care for hospitalized patients within their panel
• Contribute to practice efficiency, quality initiatives, and clinical workflows

Qualifications

• Master's degree and national certification as a Nurse Practitioner (MSN, ANCC/AANP) or Physician Assistant (Master’s, NCCPA)
• Active, unrestricted Massachusetts NP or PA license, DEA, and BLS certification
• Minimum of three years of independent primary care experience managing a full patient panel
• Experience with preventive, longitudinal, and chronic care, especially for adult patients aged 65+

Skills

• Experience in value-based or population health models
• Managing complex adult patients with multiple chronic conditions
• Team-based care experience with interdisciplinary support
• Familiarity with clinical pathways, quality initiatives, and Epic EHR
