Primary Care Physician, Pod Leader

As part of our mission-driven interdisciplinary team, our Primary Care Physicians can build deeper, long-term relationships with fewer patients, allowing you to see the impact of your care every day.

Responsibilities

• Perform comprehensive in-person assessments, leading and partnering with the interdisciplinary team (IDT) to develop a personalized care plan for each participant
• Coordinate discharge for acute, short-term, and long-term placement
• No hospital rounds – focus on comprehensive care, seeing an average of 8 patients a day

Requirements

• Bilingual in Spanish & English is required
• Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
• Masters or doctorate degree
• Board Certification preferred
• Active state physician license without restrictions
• State CDS certification and DEA registration
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

Qualifications

• Eligible for enrollment to provide services to Medicare and Medi-Cal participants
• Board certification preferred
