Primary Care Physician-OSH

We are seeking a Primary Care Physician to deliver equitable, value-based healthcare to Medicare populations in neighborhood clinics. Key responsibilities include assessing, diagnosing, and managing patient care, prescribing medications, maintaining electronic medical records, collaborating with leadership to meet quality goals, and guiding care teams. Qualifications require an M.D. or D.O., board certification or eligibility in Internal Medicine or Family Medicine, an active unrestricted license, DEA license, and US work authorization. Preferred skills include geriatrics fellowship, experience with older populations, outpatient primary care, and familiarity with value-based care models. Bilingual abilities are a plus.
