About the role
Primary Care Physician responsible for providing equitable, value-based healthcare to Medicare patients in a community-focused setting. Operates within a single medical center supported by care teams to prioritize quality over volume, aiming to improve patient outcomes, reduce hospital admissions, and deliver preventive care. Collaborates with regional leadership and practice managers to meet health quality goals and oversee care delivery.
Responsibilities
- Assess, diagnose, and treat patients
- Prescribe medications and manage pharmaceutical treatments
- Maintain electronic medical records
- Lead and coordinate care teams
- Work with leadership to achieve health quality objectives
- Perform other assigned duties
Qualifications
- MD or DO degree with board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state license and DEA license
- US work authorization
Preferred Skills
- Fellowship in Geriatrics and experience with older adult populations
- Experience in value-based care models and outpatient primary care
- Bilingual proficiency where applicable
- Collaborative approach to achieve positive health outcomes
