Role Overview We are seeking a Primary Care Physician (MD/DO) who owns an existing clinic to provide in-facility care to residents in assisted living communities across the Inland Empire, CA. This role involves mobile in-facility visits with clustered routes to minimize travel time. This is a part-time position starting at a minimum of 1 day per week (Monday through Thursday), with potential to expand based on patient panel growth. This position is available as W-2 or 1099 — we are happy to accommodate what works best for you. Responsibilities Provide comprehensive primary care to seniors in assisted living and RCFE communities in the Inland Empire areaConduct in-facility patient visits and telehealth consultations as neededManage chronic conditions, preventive care, and acute concernsCollaborate with our care team including NPs, specialists, and facility staffMaintain accurate and timely documentation in compliance with regulatory requirements Requirements Must own an existing clinic contracted with IEHP (Inland Empire Health Plan)Active California medical license (MD or DO) in good standingBoard Certified or Board Eligible in Internal Medicine, Family Medicine, or GeriatricsActive DEA registrationMedicare credentialing preferredExperience in geriatric or senior care settings preferredValid driver's license and reliable vehicle Compensation This is a part-time position starting at 1 day per week with potential to expand based on patient panel growth. Compensation is $800 per day, plus $50 per patient seen.
