About the Role
The Primary Care Physician provides equitable, value-based healthcare to Medicare patients in neighborhood clinics. Operating within a collaborative team, they focus on delivering high-quality, preventive care aimed at reducing hospital admissions and enhancing patient outcomes.
Responsibilities
- Assess, diagnose, and manage patient health.
- Prescribe treatments and maintain electronic medical records.
- Collaborate with leadership and care teams to meet health quality goals.
- Oversee patient care and medication management.
Requirements
- MD or DO degree.
- Board Certified or eligible in Internal Medicine or Family Medicine.
- Active, unrestricted State License and DEA certification.
- US work authorization.
Qualifications
- Geriatrics fellowship (preferred).
- Experience with older populations, value-based care, or outpatient settings (preferred).
- Bilingual skills (preferred).
Schedule
Full-time position with an anticipated weekly effort of 40 hours.
Pay
Compensation ranges from $174,070 to $374,920, based on experience and location.
