Primary Care Physician

Responsibilities

• Deliver acute and chronic illness management
• Assess, diagnose, treat, and coordinate patient care
• Lead care teams
• Engage with hospitalized and homebound patients
• Promote health and behavior change
• Ensure compliance with standards
• Contribute to center operations, recruitment, and culture

Qualifications & Skills

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar
• Active State Medical License
• Board eligibility or certification
• BLS certification
• Proficiency in evidence-based medicine, leadership, quality improvement, and technology
• Excellent communication, accessibility, and self-care practices
• Flexible availability including evenings and weekends
• Able to travel locally and nationally

• Manage up to 450 patients
• Meet clinical and performance targets

• Supervise APPs and maintain high standards of care in a compassionate, efficient manner
