Responsibilities
- Deliver acute and chronic illness management
- Assess, diagnose, treat, and coordinate patient care
- Lead care teams
- Engage with hospitalized and homebound patients
- Promote health and behavior change
- Ensure compliance with standards
- Contribute to center operations, recruitment, and culture
Qualifications & Skills
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar
- Active State Medical License
- Board eligibility or certification
- BLS certification
- Proficiency in evidence-based medicine, leadership, quality improvement, and technology
- Excellent communication, accessibility, and self-care practices
- Flexible availability including evenings and weekends
- Able to travel locally and nationally
Key Metrics & Duties
- Manage up to 450 patients
- Meet clinical and performance targets
- Foster a positive culture
- Collaborate with healthcare teams
- Participate in quality programs
- Supervise APPs and maintain high standards of care in a compassionate, efficient manner
