About the Role
The Primary Care Physician, Partner (PCPP) provides comprehensive care for adult patients, managing acute and chronic illnesses with accountability for outcomes and cost-effectiveness. They lead care teams, coordinate with specialists, and engage in health coaching to promote behavior change and improve patient health.
Responsibilities
- Independently assess, diagnose, treat, and document patient conditions.
- Lead and collaborate with care teams, including care promoters, facilitators, and coordinators.
- Manage patients across various settings, including hospital, SNF, LTC, or home-based care.
- Ensure compliance with clinical guidelines, quality standards, and departmental goals.
- Participate in risk management, clinical meetings, and center operations, including recruiting efforts.
- Supervise Advanced Practice Practitioners, maintaining accountability for their actions.
Requirements
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar, with active licensure.
- Minimum of 6 years clinical experience, including 2 years in a senior PCP role.
- Board certification or eligibility, DEA license, and BLS certification.
- Strong leadership, communication, and evidence-based practice skills.
- Proficiency in Microsoft Office and willingness to travel locally and nationally as needed.
- Fluency in English; additional languages may be required depending on location.
