About the role
ChenMed is a physician‑led, mission‑driven primary care organization focused on delivering affordable VIP care to seniors. The Primary Care Physician (PCP) is accountable for outcomes, strong clinical care, and cost‑effectiveness for a panel of up to 450 patients. The PCP acts as a health coach, simplifies care plans for actionable behavior change, and helps champion a culture of love, accountability, and passion.
The organization measures metrics such as patient admissions per thousand, appointment utilization (18‑21 slots per day), CGCAHPS patient experience, clinical gap closures, and medical cost measures. PCPs are expected to meet these targets in partnership with center and market leadership.
Responsibilities
- Provide independent care for older adult patients with acute and chronic illnesses.
- Take full accountability for patient care and outcomes, consulting specialists as needed while remaining involved in detailed care.
- Engage with hospitalists for any patient hospitalized, regardless of the hospitalist’s affiliation.
- Perform assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for acute and complex chronic health needs.
- Lead a care team that includes a care promoter (medical assistant), care facilitator, and care coordinator for office‑based patients.
- Coordinate with transitional‑care teams, case managers, acute and transitional‑care physicians, and other resources for patients unable to visit the office (hospital, SNF, LTC, homebound).
- Participate in center management, cover for other providers as needed, and assist in recruiting patients and additional providers.
- Supervise, collaborate with, or function within a practice agreement with an Advanced Practice Practitioner (APP) and remain accountable for the APP’s actions.
- Participate in risk and quality management programs, clinical meetings, and other meetings that promote patient health and company goals.
- Adhere to departmental goals, performance standards, regulatory compliance, quality patient‑care compliance, and company policies and procedures.
- Perform other duties as assigned by management.

- Willingness to travel locally as needed, nationally for initial training (2 weeks), and occasionally for recruiting or training.
- Fluency in English (verbal and written); additional language requirements will be noted per center.
- Independent judgment and flexibility to work evenings, weekends, and/or holidays as needed.
Education and Experience
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Ability to obtain or hold a current, active State Medical License for the state(s) of practice.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred; board eligibility required.
- Commitment to maintain board certification through MOC, CME, or re‑examination as required.
- Current DEA number for Schedule II‑V controlled substances.
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within the first 90 days of employment.
Pay
Base annual salary range: $221,141 – $315,915. Final compensation depends on experience, education, geographic location, and other relevant factors. The position may also be eligible for bonuses or commissions.
