About the Role
The Primary Care Physician (PCP) in our organization demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence patient outcomes by building trusting relationships and helping patients change behaviors.
- Coaching for health: The PCP acts as a health coach, helping patients set short and long-term health goals, partnering with them to work toward those goals, and frequently following up on progress.
- Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for patients and when leading care teams toward performance goals.
We are an outcomes-focused, value-based organization. For their panel of patients, the following metrics are regularly measured to help PCPs succeed and reach partnership status:
- Patient admissions per thousand
- 18-21 appointment slots per day (new patients count as 2 slots, follow-ups as 1)
- CGCAHPS (patient experience scores)
- Clinical gaps closures
- Medical cost measures
Each PCP will have goals for these metrics and will work toward them with center and market leadership as well as their care teams. Culture is vital in our medical centers, and as leaders, PCPs are expected to champion a positive culture of love, accountability, and passion alongside center leadership.
Responsibilities
- Independently provide care for patients with acute and chronic illnesses, primarily older adults.
- Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining involved in detailed patient care.
- Engage with hospitalists whenever a patient is hospitalized, regardless of whether the hospitalist works for ChenMed.
- Assess, diagnose, treat, manage, educate, promote health, and coordinate care for patients with acute and complex chronic health needs.
- Lead a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to visit the office.
- For patients unable to come to the office (hospital, SNF, LTC, or homebound), engage with the transitional care team, case managers, acute and transitional-care physicians, and other available resources.
- Play an active role in center management, covering for other providers when needed, and assisting in recruiting patients and additional providers.
- Adhere to departmental goals, performance standards, regulatory compliance, and quality patient care policies.
- Perform other duties as assigned.

- Quality: Work with care teams to achieve high-quality outcomes and measures while seeking ways to improve medical practice and center operations.
- Influence: Competently and compassionately influence patients, teams, and yourself to achieve the best outcomes.
- Self-Care: Prioritize your own physical, mental, social, and spiritual well-being to sustainably care for patients and promote a healthy work environment.
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other relevant software.
- Ability and willingness to travel locally, nationally for initial training (2 weeks), and occasionally regionally/nationally for recruiting or training.
- Fluency in English (verbal and written); additional languages may be required in some centers.
Requirements
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
- Current, active State Medical License for the state(s) of practice (or ability to obtain one).
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred; Board Eligibility required.
- Maintain board certification through MOC, CME, or retaking board exams as required.
- Current DEA number for schedule II-V controlled substances.
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross (within first 90 days of employment).
Pay
The pay range for this position is $221,141 - $315,915 annually. Final compensation will depend on experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
