Primary Care Physician

Responsibilities

• Provide comprehensive care for patients with acute and chronic conditions, ensuring positive outcomes and cost-effectiveness.
• Lead clinical teams, conduct weekly meetings, support center management, and foster a positive, team-oriented culture.
• Engage in patient health coaching, simplify care plans, and actively participate in quality and risk management programs.
• Collaborate with hospital and transitional care teams, and assist in recruiting patients and providers.
• Maintain accessibility for patients and promote evidence-based, lifestyle, and nutrition interventions.

Qualifications & Skills

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar
• Active or able to obtain State Medical License
• Board certification or eligibility in relevant specialty
• Current DEA license and BLS certification
• Strong leadership, communication, and interpersonal skills
• Commitment to high-quality, patient-centered care
