About the Role
The Primary Care Physician (PCP) provides comprehensive care for older adult patients with acute and chronic conditions, ensuring quality outcomes and cost-effective treatment.
Responsibilities
- Independently assess, diagnose, treat, and manage patient health.
- Collaborate with specialists and care teams for in-hospital, SNF, LTC, or homebound patients.
- Lead and coordinate care teams, promote health goals, and engage in care transitions.
- Maintain compliance with clinical guidelines, quality standards, and organizational policies.
- Support center operations, participate in clinical meetings, and contribute to a positive, patient-centered culture.
- Demonstrate physician leadership, influence positive behaviors, and foster teamwork.
- Supervise Advanced Practice Providers (APPs), participate in recruitment, and contribute to center management.
- Exercise independent judgment and work flexibly, including evenings, weekends, or holidays as needed.

Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related field.

- DEA registration and BLS certification required.
- Must meet educational and experience criteria as specified.
