About the Role

The Primary Care Physician (PCP) provides comprehensive care for older adult patients with acute and chronic conditions, ensuring quality outcomes and cost-effective treatment.

Responsibilities

• Independently assess, diagnose, treat, and manage patient health.
• Collaborate with specialists and care teams for in-hospital, SNF, LTC, or homebound patients.
• Lead and coordinate care teams, promote health goals, and engage in care transitions.
• Maintain compliance with clinical guidelines, quality standards, and organizational policies.
• Support center operations, participate in clinical meetings, and contribute to a positive, patient-centered culture.
• Demonstrate physician leadership, influence positive behaviors, and foster teamwork.
• Supervise Advanced Practice Providers (APPs), participate in recruitment, and contribute to center management.
• Exercise independent judgment and work flexibly, including evenings, weekends, or holidays as needed.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related field.
• Active medical license and board eligibility or certification.
• DEA registration and BLS certification required.
