Primary Care - Physician Assistant or Nurse Practitioner

About the role

Located among the beautiful hills and quaint towns of northwestern Connecticut, join our primary care team in Litchfield!

We're seeking a compassionate and collaborative primary care Nurse Practitioner or Physician Assistant. You'll have the opportunity to build your own patient panel in a patient-centered environment, with ample support for seamless integration.

Responsibilities

• Perform routine physical examinations
• Formulate medical, educational, and psychosocial plans to treat patients
• Develop plans for further evaluation of health needs, including referrals to other health team members or specialists
• Provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols
• Identify problems outside of their scope of practice and refer appropriately to physicians

Requirements

• Licensure: Unrestricted State of Connecticut Medical License (or eligible)
• Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
• Experience: 2+ years’ experience in a primary care or relevant outpatient setting
