About the role
The Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) plays a vital role in delivering high-quality, patient-centered care for women aged 65+.
Responsibilities
- Evaluate patients' acute and chronic conditions
- Manage medications, mental health, and social-emotional needs
- Educate patients on health maintenance and disease prevention
- Collaborate with the care team to assess, plan, and provide personalized care
- Ensure adherence to quality standards aligned with the organization's values
Requirements
- Valid NP (FNP-C or AGNP) or PA (PA-C) license
- 3-5 years of experience managing complex diseases (e.g., diabetes, cardiovascular issues, osteoporosis)
- Experience with geriatric and women's health
- Poly-pharmacy management and physical/gynecological exams
- Strong problem-solving skills and adaptability
- Knowledge of medical advancements
- Meet certification, licensing, vaccination, and legal requirements, including DEA and NPI credentialing
